COMMUNITY SUPPORTS

Let us be your advocate for community supports.  As our health changes, so do our needs.  With ongoing assessments, we will refer through the community for OT's, dieticians, Physio Therapists, spiritual services and equipment needs.  Let us advocate for our Palliative Care Outreach Team involving Nurse Practitioners and Palliative Physicians at your bedside.  As an effective team we work closely with your family doctor to provide optimal care.  Resources and communication are key.
